For Oregon youth aging out of foster care, the Chafee Medical Program provides medical coverage. To qualify, you must have been receiving foster care medical when you turned age 18. The Chafee Medical Program, upon qualifying, will provide you with medical coverage up to your 21st birthday. If you are still in the custody and care of DHS, you may contact your DHS worker to see if you are eligible for Chafee Medical. The Chafee Medical Program Referral form should be in your Transition Tool Kit check list. You will fill it out with your caseworker. If you need a copy of the form, click here.
